Taxi Cab License

I, the undersigned, hereby make application for license to operate a taxicab or taxicabs upon the streets of the City of Madison, South Dakota, for hire and state as follows: That is application is made pursuant to Chapter 21 of the 1979 Revised Ordinances of the City of Madison; and That the following information is true to the best of my knowledge, information and belief:

  • License fee tendered herewith: ($25.00 first and second taxicab or bus and $25.00 for each additional taxicab to be paid at City of Madison Office or mailed to 116 W. Center Street Madison, SD 57042.
  • Each applicant for the license required by this chapter shall file a bond in the sum of five hundred dollars ($500.00) and also public liability and property damage insurance to cover each taxicab or bus for at least fifty thousand dollars ($50,000.00) property damage and at least one hundred thousand dollars ($100,000.00) public liability for any one person and two hundred thousand dollars ($200,000.00) public liability for more than one person.
  • The operator will hold and save the City of Madison harmless and indemnify the City of Madison, against and from all claims, liens, judgments and costs incurred by the City of Madison of any kind or any reason, including but not limited to passenger of said taxi service resulting from the operation of said taxi service.
  • It is further agreed by me that upon tentative approval your applicant will furnish any additional information necessary and not here supplied, for final approval and granting of license.
